While under the care of Queen City Homecare, a nurse will visit your home to ensure that the proper services are being provided and you or your loved one as both our patient and customer, are satisfied with the care being given. These continued nursing assessments help to ensure that your initial In Home Assessment was correct and is accurately being monitored and carried out. If you determine after your In Home Assessment that Queen City Homecare is an appropriate fit for your needs, we offer various services to assist in your care, available 24 hours a day, 7 days a week.

Private duty care can be costly, but at Queen City Homecare, we allow our patients the option of splitting their blocked time with other Queen City Homecare patients within a one mile radius. Perhaps you need a limited amount of time and a neighbor also needs a limited amount of time. By splitting your time with our skilled staff, both parties can save money and still receive the care desired.
Our special disease based care plans are specifically tailored to address yours and your loved one’s unique needs. Each package is created by a skilled home care nurse who knows what tools are frequently required by similar patients experiencing the same diagnosis, for example complimentary diary charts. Queen City Homecare has available for sale monitoring tools supplemental to these packages such as blood pressure cuffs, alarming mats, alarming medication sets, and scales to help track the disease progress and to assist family and medical personnel in your care.

THE SUCCESSFUL RECOVERY PLAN
The Successful Recovery Plan is a 4 step process:
- Step 1: When the client is ready for discharge from the hospital a Queen City Homecare nurse will speak with a member of the hospital staff to create a Plan of Care for the client.
- Step 2: A Queen City Homecare nurse will meet with the client for the initial home visit to explain the Plan of Care to not only the client, but also the client’s support group. The nurse will do a complete assessment at that time, answer any questions and gather any other pertinent information.
- Step 3: A fully trained LPN, STNA or CNA will be involved with the client’s care as directed. This will help to ensure that the client follows the individualized plan of care.
- Step 4: A report will be received by a nurse on a regular basis to catch warning signs and inform the physician’s office in a timely manner.
